UIDAI Form 2024
The Unique Identification Authority of India (UIDAI) is responsible for issuing Aadhaar numbers, which serve as unique identity numbers for residents of India. To apply for an Aadhaar card, you need to fill out the UIDAI application form and provide supporting documents.
UIDAI Application Form
The UIDAI form, also known as the Aadhaar Enrollment/Correction Form, is used for:
- New Aadhaar enrollment.
- Updating or correcting existing Aadhaar details.
How to Fill the UIDAI Form 2024
- Form Availability: The form can be downloaded from the UIDAI website or obtained at an Aadhaar enrollment center or directly downloaded from the link given inside this article.
- Sections of the Form:
- Pre-enrollment ID: If you have pre-registered online, enter the pre-enrollment ID.
- Full Name: Enter your full name as you want it to appear on the Aadhaar card.
- Gender: Tick the appropriate box for male, female, or transgender.
- Age/Date of Birth: Enter your date of birth in DDMMYYYY format. If you don’t have proof of exact date of birth, mention your age in years.
- Address: Provide your residential address. This address will be printed on the Aadhaar card and used for correspondence.
- Email and Mobile Number: These are optional but recommended for future communication and OTP verification.
- Consent: Sign or thumb impression to consent to provide your information to UIDAI.
- Supporting Documents: Attach copies of the required documents. Original documents must be shown at the enrollment center for verification.

For new Aadhaar enrollment or updates, you need the following documents:
- Proof of Identity (PoI) (Anyone):
- Passport
- PAN card
- Voter ID
- Driving license
- Ration/PDS photo card
- Government photo ID cards/service photo identity card issued by PSU
- NREGS job card
- Photo ID issued by a recognized educational institution
- Arms license
- Photobank ATM card
- Photo credit card
- Pensioner photo card
- Freedom fighter photo card
- Kissan photo passbook
- CGHS/ECHS photo card
- Address card having name and photo issued by the Department of Posts
- Certificate of identity having a photo issued by a gazetted officer or Tehsildar on letterhead
- Proof of Address (PoA) (Anyone):
- Passport
- Bank statement/passbook
- Post office account statement/passbook
- Ration card
- Voter ID
- Driving license
- Government photo ID cards/service photo identity card issued by PSU
- Electricity bill (not older than 3 months)
- Water bill (not older than 3 months)
- Telephone landline bill (not older than 3 months)
- Property tax receipt (not older than 3 months)
- Credit card statement (not older than 3 months)
- Insurance policy
- Signed letter having photo from bank on letterhead
- Signed letter having photo issued by a registered company on letterhead
- Signed letter having photo issued by a recognized educational institution on letterhead
- NREGS job card
- Arms license
- Pensioner card
- Freedom fighter card
- Kissan passbook
- CGHS/ECHS card
- Certificate of address having photo issued by MP or MLA or gazetted officer or Tehsildar on letterhead
- Vehicle registration certificate
- Registered sale/lease/rent agreement
- Address card having photo issued by the Department of Posts
- Caste and domicile certificate having photo issued by the State Government
- Proof of Date of Birth (Any one):
- Birth certificate
- SSLC book/certificate
- Passport
- Certificate of date of birth issued by Group A gazetted officer on letterhead
