Ration Card Form (No. 14) West Bengal

This Form can be used for shifting of address from one place to another due to marriage/divorce and any other reasons. The Food Supply and Consumer Affairs Department of the West Bengal government has released the application for a ration card. A resident of West Bengal can apply online for a new digital ration card on the West Bengal Public Distribution System (WBPDS) website.

How to Fill the Form No 14 Ration Card
- Existing Card type and number of the person shifting into a new Family
- Existing Card type and number of the Head of Family / Any other member of the family where Applicant wants to shift
- Reason for shifting to a New Family
- Contact details of the family (to which the applicant is shifting )
- Aadhaar Details of all existing DRC holders of the New Family
- Address details
- And any other details
Documents Required for Ration Card Form 14 West Bengal
- Copy of Digital Ration Card of Head of Family / one existing cardholder
- Copy of Aadhaar documents of the applicants
- Marriage (Proof of Marriage & document/s establishing a relationship with the family to be attached)
- Divorce /Separation (Proof of Divorce to be attached)
- If the age of the applicant is less than 5 years, then a copy of the Aadhaar card is not mandatory. In that case, copy birth certificate of such an applicant is to be submitted
- Documents establishing a relationship with the family to be attached.
- Copy of DRC of the HOF and the husband/father/guardian of the new Family.
- Another reason (Declaration with the signature of the member of the new family needed)
