PF Joint Declaration Form 11
EPF Joint Declaration Form 11 is used for the declaration of an employee for both provident fund and pension scheme. Any individual taking up employment in an organization in which the Employees’ Provident Fund and Family Pension Scheme are in force has to fill this form as a mandate.
How to Fill PF Declaration Form 11
- You can usually download the PF Joint Declaration Form 11 from the official website of the Employees’ Provident Fund Organisation (EPFO) or obtain it from your employer’s HR department.
- Fill in your details such as your name, father’s name/husband’s name, date of birth, and PF account number.
- Provide your current employment details including establishment name, establishment code, your employee/member ID, and your UID number (if applicable).
- If you’re declaring previous employment details for transfer or consolidation of PF accounts, provide the name and address of your previous employer, the PF account number with that employer, and other relevant details.
- Once you’ve filled out the form, submit it to your current employer’s HR department for verification and further processing. Your employer may verify the details provided and attest the form accordingly.
- After verification, your employer will submit the form to the EPFO office for further action.
