Who Can Use Form DS-82? (Eligibility Criteria)
You can renew your passport using Form DS-82 if:
- Your most recent passport was issued when you were 16 years or older
- Your passport was issued within the last 15 years
- Your passport is undamaged and in your possession
- Your passport was issued in your current name, or you have legal proof of name change
Required Documents for Renewal
To successfully renew your passport, you must include:
- Completed DS-82 Form – Download it from travel.state.gov
- Your Most Recent Passport (original, undamaged)
- One Passport Photo – 2×2 inches, white background, no glasses
- Name Change Document (if applicable) – Marriage certificate, court order, etc.
- Payment – Check or money order payable to “U.S. Department of State”
