A Medical Leave Form Certificate is essential for any employee seeking medical leave. This certificate is a written statement from a physician that confirms the results of a medical examination of a patient issued by the Doctor. Employees are eligible for **one month of medical leave** for every 18 months of service, and this is compensated at half the wages. Additionally, any person covered under the Act can avail themselves of sick leave, not less than one-eighteenth of their service period, also at half the wages.
Eligibility for Medical Leave
To qualify for this medical leave, the individual must have served the company for at least 12 months. This duration can be extended if the employee is under medical care or has been hospitalized. If the medical leave is for a family member, the patient must have been in the facility for at least 3 consecutive days. An application for medical leave in the case of pregnancy should also be submitted to the office.
Special Cases
In situations involving chronic diseases, the company is required to provide two master reviews. If an employee faces complications such as abortion, the medical records of the patient, including the number of check-ups and any related births, must be appropriately compensated by the company.
