मी श्री अर्जदाराचे वडिल किंवा पालक यांचे पूर्ण नाव रा. गावाचे नाव लिहावे ता. तालुका लिहा. जि. जिल्हा लिहा येथील कायम रहिवासी असुन मला एकुण अपत्य (मुलांची संख्या येथे भरावी) अपत्य आहेत. त्यापैकी मुले संख्या भरा व मुली मुलींची संख्या लिहा आहेत.
अर्जदाराचे नाव लिहा____ हा प्रथम/ द्तीविय / तृतीय / चतुर्थ येथे किती नंबर चे अपत्य आहे ते लिहा क्रमांकाचा लाभार्थी अपत्य (पुरुष/ स्त्री) gender लिहा आहे. तो/ती ____________या महाविद्यालय /विद्यालय मध्ये _महावीद्यालायाचे नाव लिहा या __ कोर्स चे नाव अभ्यासक्रमास शिक्षण घेत असुन तो/ती ____अर्जदाराने अप्लाई करावयाची योजनेचे नाव लिहा या शिष्यवृत्ती योजने करीता अर्ज करीत आहे.
यापूर्वी माझ्या कुटुंबातील एकूण __ किती अपत्यं या योजने साठी लाभ घेतला आहे ती संख्या लिहा (पुरुष/ स्त्री) शिष्यवृत्तीचा लाभ घेतला आहे त्यांची नावे.
