LIC NEFT Mandate Form
LIC NEFT Form is used by policyholders to authorize LIC (Life Insurance Corporation of India) to credit their policy maturity or claim payments directly into their bank account through the NEFT (National Electronic Funds Transfer) system
How to Fill Up LIC NEFT Mandate Form
- Policyholder Information: Fill in your policy number, name, address, and contact details accurately. Ensure that the information matches the details mentioned in your LIC policy documents.
- Bank Account Details:Provide your bank account details, including the bank name, branch address, account number, type of account (savings, current, etc.), and IFSC (Indian Financial System Code) code.
- Policy Details: Mention the policy numbers for which you are authorizing LIC to credit payments via NEFT. If you have multiple policies, ensure that you list all the relevant policy numbers accurately.
- Signature: Sign the mandate form in the designated space provided. Your signature confirms your consent to authorize LIC for NEFT payments related to your policy.
- Submission:Once you have filled out the form completely and accurately, submit it to your nearest LIC branch office or insurance agent. Make sure to attach any supporting documents requested by LIC, such as a canceled cheque or bank account statement.
- Verification: After submission, LIC may verify the provided bank account details for authenticity and accuracy. This verification process ensures that payments are credited to the correct bank accounts.
- Confirmation:Once the NEFT mandate is successfully registered with LIC, you will receive confirmation either through email, SMS, or postal mail.
Documents required for LIC NEFT Form –
- Cancelled cheque leaf with name and account number printed (or)
- Cancelled cheque leaf along with a photocopy of the page of Bank passbook containing the details of the account holder if name and account number not printed on the cheque leaf.
