1. Obtain the Form

2. Complete the Form
You will need to provide the following information:
- Patient’s Information: Full name, date of birth, address, and contact details.
- Recipient’s Information: Name of the individual(s) or organization authorized to receive the information.
- Type of Information Released: Specify what health records can be shared (e.g., medical history, test results, mental health information).
- Purpose of Release: Why the information is being shared (e.g., legal purposes, family care, insurance claims).
- Expiration Date: Set a date when the authorization will expire, if applicable.
3. Sign and Date the Form
- The patient (or their legal representative) must sign and date the form.
- If a legal guardian or power of attorney is signing, they must provide documentation proving their authority.
4. Submit the Form
- Deliver the completed form to the healthcare provider either:
- In-person
- By mail
- Electronically, if allowed by the provider
5. Processing Time
- Processing can take a few days to a couple of weeks, depending on the provider’s policies.
- Keep a copy for your records.
6. Revoking Authorization
- You can revoke the authorization in writing at any time by notifying the healthcare provider.
