EPFO Form 10D
This is an application form for a monthly pension under the Employees’ Pension Scheme (EPS), provided by the EPFO department. All members become eligible for a pension upon retirement at the age of 58 years. However, the member can choose to receive a reduced pension starting at age 50, with a discount of 4% for each year before 58. To apply for a monthly pension, you need to fill out the EPF Form 10D.

Documents Required to be Enclosed with EPF Form 10D
- Descriptive role of pensioner and his/her specimen signature/Thumb impression (in duplicate) (this form is included with the Claim Form).
- 3 passport-size photographs.
- If there is permanent and total disablement, the employee must undergo a complete medical examination by the Medical Board appointed by the EPFO. Relevant documents should be attached with the form.
- The establishment must provide the certificate and wage details of the member at the time of retirement or death.
- If the establishment is closed and no authorized officer is available, the application should be forwarded through any person from a list that includes a magistrate, gazetted officer, bank manager, or any other authorized personnel approved by the Commissioner.
