How to Get Disability Certificate in Kerala
1. Obtain a Medical Diagnosis:
- Visit a government-recognized hospital or a specialist doctor to get a diagnosis of your disability.
- The doctor will provide you with a medical report stating the nature and extent of the disability.
2. Fill the Application Form:
- Obtain the application form for the disability certificate. You can get this from the District Medical Board office or download it from the Kerala government’s official website.
3. Submit the Required Documents:
- Attach the following documents to your application:
- Medical reports and prescriptions from the doctor.
- Proof of identity (Aadhaar card, Voter ID, etc.).
- Proof of residence (Ration card, Utility bills, etc.).
- Passport-sized photographs.
- Any other document as specified by the authorities.
4. Submit the Application:
- Submit the filled application form along with the required documents to the District Medical Board in your area. You can either submit it in person or, in some cases, apply online through the Kerala State Disability Commission website.
