- Details of Nominee(s):
- Name(s) of the nominee(s)
- Relationship with the subscriber
- Age of the nominee(s)
- Full address of the nominee(s)
- Proportion of the GPF amount to be paid to each nominee
- Declaration by the Subscriber:
- Statement declaring the nomination
- Date and place of signing
- Signature or thumb impression of the subscriber
- Witness Section:
- Names and addresses of two witnesses
- Signatures of the witnesses
- Office Use Section:
- Verification by the office/department
- Signature and designation of the verifying officer
- Date of verification
