LIC Maturity Form
LIC Maturity Form can be downloaded from the official website of LIC of India i.e https://licindia.in or it can be directly downloaded in PDF format from the link given at the bottom of this page.
Please submit your Discharged Receipt in Form No.3825 with the original policy document at least one month before the due date so that the payment is received before the due date of the maturity claim.
How to Fill Up LIC Maturity Form

- The LIC maturity form, also known as the “Discharge Voucher” or “Claim Form No. 3825,” can be obtained from the LIC branch office or downloaded from the LIC website.
- Fill in Personal Details:
- Policy Number: Write your LIC policy number.
- Name of the Policyholder: Enter your full name as mentioned in the policy document.
- Address: Provide your current residential address.
- Policy Details:
- Date of Maturity: Enter the date on which the policy matures.
- Sum Assured: Write the sum assured under the policy.
- Name of the Branch: Mention the name of the LIC branch where the policy is serviced.
- Bank Account Details:
- Bank Name: Enter the name of your bank.
- Branch Name: Write the branch name of your bank.
- Bank Account Number: Provide your bank account number where the maturity amount will be credited.
- IFSC Code: Mention the IFSC code of your bank branch.
- Documents Attached:
- Check the list of documents you are attaching with the form, such as the original policy document, identity proof, address proof, and bank passbook/cancelled cheque.
- Signature:
- Policyholder’s Signature: Sign the form where required. Make sure your signature matches the one on the policy document and other records.
- Witness Signature: A witness, who is not a family member, should also sign the form with their name and address.
- Submit the Form:
- Submit the duly filled form along with the required documents to the LIC branch office where your policy is serviced.
- Acknowledgement:
- Obtain an acknowledgement receipt from the LIC branch for the submitted documents.
Required Documents for LIC Maturity Form
- Original Policy Document: The original policy document needs to be submitted with the form.
- Identity Proof: A copy of your identity proof such as Aadhar card, PAN card, voter ID, or passport.
- Address Proof: A copy of your address proof such as an Aadhar card, utility bill, or bank statement.
- Bank Details: A cancelled cheque or a copy of the first page of your bank passbook containing your account number and IFSC code.
- Policy Discharge Form: The filled and signed LIC maturity form (Discharge Voucher).
