Aadhaar Form 5-18 is required to update the Aadhaar card of the child who age is between 5 to 18 years. 5 वर्ष से कम उम्र के बच्चों के मामले में, माता-पिता या अभिभावक में से किसी एक का “नाम” और “आधार नंबर” अनिवार्य है। बच्चों का नामांकन करते समय माता-पिता/अभिभावक को अपना आधार पत्र प्रस्तुत करना होगा (या उन्हें एक साथ नामांकित किया जा सकता है)

The UIDAI enrolls residents of all ages, including infants. However, biometric information is not collected for children under the age of 5 years. For children under 5 years of age, their Aadhaar is linked to one of their parents or guardians. Such children need to submit their biometric information (Photograph, ten Fingerprints, and two Irises) once they attain the age of 5 years. These biometrics need to be updated again when they attain the age of 15 years.

Documents Required for Aadhaar Form 5-18

  • Birth Certificate issued by Authorized Authority (in the respective States) under the Registration of Births and Deaths Act, 1969 read with Registration of Births and Deaths Rules 1999/2000/2002 of respective States
  • The child’s birth certificate or school certificate.
  • The parent’s Aadhaar card or any other valid government ID proof such as PAN card, passport, or driving license.
  • The parent’s address proof such as electricity bill, water bill, or telephone bill.
  • A passport-sized photograph of the child.

How to Apply Aadhar Card for 5 Year Old Child Online

  • Visit the Enrolment Centre: After you have located the nearest enrolment center, visit it with the child and all the required documents. You will be required to fill out an enrolment form that will ask for personal details of the child such as name, date of birth, and address.
  • Data Collection: After filling out the enrolment form, the child’s photograph is taken. Fingerprints and iris scans are not captured while enrolling children below 5 years of age. Once the child attains the age of 5, biometrics need to be updated mandatorily.
  • If a child is below 5 years old, one of the parents or guardians has to authenticate on behalf of the child and also give consent for the enrolment of the minor by signing the enrolment form, according to UIDAI.
  • UIDAI said that if the child is an NRI, then a valid Indian passport of the child is mandatory as Proof of Identity (PoI). It also said that if the child is an Indian resident, then any valid proof of relationship documents such as a birth certificate, along with the Aadhaar of the parent or guardian can be used for enrolment.
  • Acknowledgment Slip: After the biometric data has been collected, an acknowledgment slip will be provided to you. This slip contains the enrolment ID which can be used to track the status of the Aadhaar card application.
